About the team 

AccessCare supports older people, people with disabilities, and those with complex needs to live safely and independently at home. Funded by the Commonwealth and Victorian governments, AccessCare provides community care, case management, and support programs across the municipalities of Kingston, Bayside, Glen Eira, Stonnington, and Port Phillip. AccessCare is committed to setting the benchmark for quality and innovation in community care.

About the role

Working closely with the AccessCare Manager, Council’s Executive team and key stakeholders, you will support the ongoing implementation and embedding of processes, systems and service models aligned with the Support at Home program.

You will play an important role in strengthening operational processes, supporting teams to adapt to sector reforms, and identifying opportunities to improve service delivery, client experience and organisational performance.

This role will focus on continuous improvement, service innovation and ensuring AccessCare is well positioned to respond to ongoing changes across the aged care sector.

Other Responsibilities include                  

  • Support the ongoing embedding and refinement of operational processes aligned with the Support at Home program.

  • Work closely with the AccessCare Manager, Executive and Council to monitor how services are performing under the new model and identify risks, opportunities and areas for improvement.

  • Lead business analysis and planning activities to strengthen service delivery and organisational readiness in a changing aged care environment.

  • Identify opportunities to enhance AccessCare’s service offering, client experience, and operational efficiency.

What You’ll Bring

  • Strong knowledge of the aged care, disability, health or community services sector, including current reforms and government directions.

  • Experience leading or supporting organisational change, service improvement initiatives, or complex projects.

  • Well-developed analytical and reporting skills, with the ability to interpret service data and support strategic decision-making.

  • Excellent stakeholder engagement and communication skills, with the ability to build strong relationships across teams and with external partners.

Qualifications and Experience

  • Tertiary qualifications in Allied Health, Behavioural Sciences or a related discipline, and/or significant experience in a similar leadership role within the community or aged care sector.
