The role

We are seeking a Vulnerable Persons Coordinator to join our team. Reporting to the Team Leader Access and Support, your role is to:

  • Supervise staff working with complex clients, particularly those experiencing homelessness or with physical and mental health conditions across HACC programs.
  • Support the Team Leader in monitoring program targets and ensuring the team meets funding requirements.
  • Manage a caseload of clients, ensuring ongoing assessment and development of person-centred care plans.

About you

We are looking for someone with:

  • Tertiary qualifications in health or social sciences
  • Strong knowledge of HACC PYP, Linkages, and CCP programs
  • Excellent interpersonal skills, with the ability to build rapport and work collaboratively with clients, carers, and service providers
  • Ability to demonstrate the City of Kingston’s values of;
