  • Join our team of tertiary qualified Care Partners in ensuring the clients on our Support at Home Program receive services tailored to support them in their goal of continuing to live independently in the community. 
  • Full time on-going opportunity

  • $100,968 - $110,162 per annum plus superannuation (dependent on experience)

About the team

AccessCare's team of Care Partners provides case management for clients in receipt of Support at Home funding.  The team is supported by dedicated Intake staff and a number of Team Support Officers, working together with our rostering staff to ensure consistent service delivery in line with best practice principles.  

About the role 

We are looking for two Care Partners to join our team. In this role, you will be responsible for managing a caseload of clients who are receiving Support at Home funding.  Your role includes on-going assessment and care planning, ensuring that services delivered meet individual requirements, supporting clients to get maximum benefit from their allocated funding and assisting them to identify and work towards their individual goals.

What you’ll deliver                      

  • Consistent case management in line with AccessCare's values and principles, and the requirements of the Strengthened Aged Care Quality Standards
  • Management of consumer budgets that reflect the consumer's care plan and goals and are maintained within required limits and guidelines
  • Responsive support that includes on-going assessment, care planning and advocacy to promote consumer access to appropriate funding resources and services
  • Strong engagement with AccessCare's teams and external partners, to promote quality service delivery and service development

What You’ll Bring/ Your Skills and Experience

  • Qualifications in the health, allied health, case management or social sciences, and proven experience in delivering high-quality case management services to people who are older and/or have a disability, preferably in delivering Support at Home packages using a brokerage model
  • Demonstrated understanding of person-centred practice and Consumer Directed Care
  • Well-developed ability to communicate with a range of different people, including colleagues, allied health professionals, clients with complex health needs and their family members and informal carers
  • Demonstrated ability to use Information Technology to support practice, including Microsoft Office, Community Care Information systems and the My Aged Care portal.
